Office Policies

Before your appointment
Please fill out the insurance and medical history forms and send them to our office via fax or e-mail. Please arrive 15 minutes before your appointment for better service. Bring any medications you take, including over the counter, prescriptions, and supplements. If you need to cancel an appointment, please call to let us know at least 24 hours in advance.

Urgent Appointments
We see patients by appointment only. Same day appointments are available. Calling ahead allows staff to verify your insurance and make sure you do not need to go the emergency room instead of the office. It also decreases wait times and insures better quality of care.

Phone messages and call backs
The office tries to return phone calls as soon as possible. The office will respond to your request as quickly as possible.

Lab and radiology results
Our goal is to provide results of your lab tests and radiology tests results in a timely fashion. The office will send an e-mail or letter explaining the meaning of your lab results. If you do not have results within 5 business days, call the office. If you have a spam blocker, please disable it so we can contact you.

Prescription refills
If you are taking a medication on a daily basis, call your pharmacy for a refill 7 days before you run out. That gives the pharmacy time to make sure your medication is in stock and send the office a fax for the doctor to review and sign. We reserve the right to ask you to make an appointment with the doctor before a medication is refilled, because certain conditions require periodic monitoring.

Referrals to specialists
When you are referred to a specialist, you will be given a referral slip by the doctor. You may need to bring certain information to the specialist’s office. It is your responsibility to make sure the specialist has that information. Call ahead of time to verify that the specialist takes your insurance and you have the necessary information to help the specialist help you.
